Privacy Policy

This notice describes how medical information about you may be used and disclosed, and how you can get access to it. Please review it carefully.

The Florissant Valley Fire Protection District is required by law to maintain the privacy of Protected Health Information (PHI) and to provide notice of our legal duties and privacy practices. This notice describes your rights and how we may use and disclose your PHI. Patient privacy is very important to the District.

How we may use and disclose your PHI

We may use and disclose your Protected Health Information for the following purposes, consistent with applicable law:

  • Treatment: to provide, coordinate, or manage your emergency medical care and related services.
  • Payment: to bill and obtain payment for the services we provide, including from your insurer or Medicare.
  • Health-care operations: to support quality assessment, training, and the general operations of the District's EMS.
  • Business associates: with outside parties, such as our billing agency, that perform services on our behalf and are required to protect your information.
  • Fundraising: as permitted by law, with the ability to opt out of such communications.
  • Persons involved in your care: with family members or others you identify as involved in your care, as appropriate.
  • Special circumstances: as required or permitted by law (for example, public-health activities, law enforcement, or legal proceedings).

Your rights

With respect to your Protected Health Information, you have the right to:

  • Receive a paper copy of this notice.
  • Access, inspect, and copy your PHI.
  • Request an amendment to your PHI.
  • Receive an accounting of certain disclosures of your PHI.
  • Request restrictions on how we use or disclose your PHI.
  • Request confidential communications by alternative means or at an alternative location.

If you have questions about this notice, want to exercise any of your rights, or believe your privacy rights have been violated, you may file a complaint with the District's Privacy Officer or with the U.S. Department of Health and Human Services, Office for Civil Rights. You will not be penalized or retaliated against for filing a complaint.
